1. Overview
CCC Strategic Solutions LLC, doing business as CCC Strategic Solutions Medical Supply ("CCC," "we," "us"), respects your privacy. This policy explains what information this website collects through each of its three forms — the quote request form, the business account application, and the supplier inquiry form — why we collect it, who can access it, how long we keep it, and how it is deleted. We collect only what's needed to respond to you.
2. Information we collect, by form
Quote request form (homepage)
Full name, organization, email address, product category, and your message describing what you need. Phone number is optional. You may also choose to opt in to occasional product updates. This form does not accept file uploads.
Business account application (Open a Business Account page)
Company name and facility type; primary contact name; accounts-payable contact (optional); email and phone; billing address and, if different, shipping address; whether your organization is tax-exempt (yes/no) and whether purchase orders are required; estimated monthly purchasing and requested payment terms; product categories of interest; what you'd like from us (for example, a credit application or catalog); and any notes you add.
This form does not accept file uploads and does not ask for tax identification numbers, W-9s, tax-exemption or resale certificates, or card or bank account information. If your account is approved and tax documentation is needed, our accounts team will request it securely during onboarding, outside this website.
Supplier inquiry form (Supplier Partnerships page)
Company or manufacturer name; website (optional); contact name and title (optional); email and phone; product categories; a description of your product line; certifications and registrations you hold (optional); and an optional file upload described in the next section.
Information collected automatically
A single local-storage entry records your cookie preference (accepted or rejected). Our hosting provider also processes standard technical data, such as IP address and browser type, as part of delivering web pages and filtering spam. This site does not currently use analytics. 3. File uploads
Only the supplier inquiry form accepts a file upload. It is optional and intended for product and company literature — a line card, capability statement, or catalog — in PDF, Word (.doc/.docx), JPG, or PNG format. Please do not upload tax forms, banking information, pricing agreements, or personal information about individuals other than business contact details. If a supplier relationship moves forward, we'll request any sensitive documents through a secure channel.
The quote request form and business account application do not accept uploads.
4. How we use it
- Quote requests: to prepare pricing and availability information and contact you about your inquiry.
- Account applications: to evaluate your application, contact you about next steps, and, if approved, set up and service your business account (including billing, shipping, and payment terms).
- Supplier inquiries: to evaluate whether your product lines fit our customer markets and sourcing standards, and to contact you about a potential relationship.
- If you opt in separately on the quote form, to send occasional product updates. This checkbox is unchecked by default, and you can unsubscribe at any time.
- To keep the site secure and functioning, including spam filtering on all three forms.
We do not use your information for targeted advertising, and we do not sell or share it with data brokers.
5. Cookies, local storage & analytics
This site does not set advertising or cross-site tracking cookies and does not currently use analytics. It uses one local-storage entry to remember whether you accepted or rejected optional third-party content (Google Fonts). If we add analytics in the future, we will update this policy, our Cookie Policy, and the consent banner before it is turned on, and it will load only with your consent.
6. Third-party services
- Netlify — hosts this website and receives and stores form submissions and supplier file uploads on our behalf (Netlify Forms). See Netlify's privacy policy.
- Our business email provider — form notifications are delivered to CCC's departmental email inboxes.
- Google Fonts — displays this site's typefaces. Sends your IP address to Google, and is requested only after you accept optional content; otherwise the site uses your device's system fonts.
We do not use any advertising pixel, analytics tool, chat widget, or session-recording tool on this site as of the date above.
7. Who can access your information
We do not sell, rent, or trade your personal information. Access is limited to:
- Quote requests: CCC sales and quoting staff.
- Account applications: CCC accounts and sales staff responsible for reviewing applications and managing customer accounts.
- Supplier inquiries and uploaded files: CCC vendor-relations and purchasing staff.
- CCC owners and administrators who manage the website and form accounts.
- Service providers who operate the site and email on our behalf (see section 6), each limited to providing that service.
- Authorities, if required by law or legal process, or to protect our legal rights.
8. Data retention

9. Deletion practices
When a retention period ends, we delete the submission and any uploaded file from our form-processing account, the email notification from our inboxes, and any copies in our working records, or anonymize it where deletion isn't practical. Copies in routine backups are overwritten in the normal backup cycle. Submissions and uploaded files are periodically cleared from Netlify once they have been transferred to our business records.
You may ask us to delete your information at any time. We'll confirm and complete the request within 30 days, unless we must keep certain records for an active account, an open transaction, or a legal, tax, or accounting obligation — in which case we'll tell you what we're keeping and why.
10. Your rights & choices
Depending on where you live, you may have the right to access, correct, delete, or receive a copy of the personal information we hold about you, and to withdraw any marketing consent or change your cookie choice (use the "Cookie Settings" link in the footer at any time). To exercise any of these, email us at the address below. We won't charge a fee for a standard request.
11. Children's privacy
This site is intended for business use by adults working in procurement, clinical, administrative, or supplier roles. We do not knowingly collect information from children, and our forms should not be used by anyone under 18.
12. Health information
None of this website's forms are designed to collect Protected Health Information (PHI) as defined by HIPAA — they ask only for business contact details and product or company information. Please do not include patient names or other patient information in any form or upload. If your relationship with CCC later involves PHI (for example, patient-specific orders), that activity is governed by separate agreements and safeguards.
13. Security
We use reasonable administrative and technical safeguards, including HTTPS encryption in transit (enforced site-wide), access-controlled form and email accounts limited to the staff described above, and spam filtering on every form. We deliberately do not accept tax documents, tax ID numbers, or payment information through this website. No method of transmission or storage is 100% secure, so we can't guarantee absolute security.
14. Changes to this policy
We may update this policy as the site changes. The "Last updated" date at the top will reflect the most recent revision, and material changes will be posted here before they take effect.
